Brief Summary:

I need an access database made into a MySQL and php plus log in. The new php site should work as the form of Access database. But it should not be possible to use the database unless one logs in.

The Access database uses "one to many" relationship which also have to be transfered to the php script and MySQL and work in the exactly same manner as the Access form.

There should be a log in screen, an overview list of the clients with search facilities. An add client screen and a client screen where one can add more informations to the animals health journal. It should be possible to edit journal entries.

The informations contained in the Access database should be imported into the new system. Maybe a script to import the database, that we could use when the project is finished, since we do use the database on a daily basis. So more informations are added to the database while this project is being done. We have to be able to update the new php script database when the work has finished.

You do not need to make any page designs for the script. Just make plain lists and forms. Nothing else.

Legal:

1) I require complete and fully-functional working program(s) in executable form as well as complete source code of all work done (so that I may modify it in the future).
2) Deliverables must be in ready-to-run condition as follows (depending on the nature of the deliverables):
2a) All other software (including but not limited to any desktop software or software the buyer intends to distribute) must include a software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request (unless specified elsewhere by the Buyer).
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
3b) No part of the deliverable may contain any copyright restricted 3rd party components (including GPL, GNU, Copyleft, etc.) unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the seller's Seller Legal Agreement.

Other Requirements:

All deliverables must be uploaded to vWorker.com before the deadline(s) for this project...with no exceptions. If this contract makes it impossible for a competent person to do this, then do not start this project...but instead alert vWorker.com of an un-arbitratable, illegal project.
